Day & Community Based Programs
Long Term Community Supports (LTCS): is a facility and community-based Medicaid In Lieu of Service (must have active NC Medicaid to be eligible)
Adult Developmental Vocational Program (ADVP): is a facility and community-based State Funded (non-Medicaid) service
Both programs focus on enhancing skills by supporting persons in small groups with pre-employment and life skills training addressing individualized goals. Participants develop skills that will assist in independence, earn money through production work, and do a variety of activities, classes, and participate in community integrated events.
Innovations Services: All services are provided through a Medicaid funded waiver, (must have active NC Medicaid and be assigned a waiver slot by an LME/MCO to be eligible)
Day Supports: is a facility and/or community-based service that can be provided in a group or on an individual basis according to the individual’s needs. Day supports involve learning independent living skills with assistance as needed with ambulation, toileting, and eating. Working on communication and behavioral management skills, self-advocacy, and participating in recreational and community activities. A variety of classes and learning opportunities are available that meet each participant's level of support needed for personal success. Regular trips are taken that can include but are not limited to parks, bowling, swimming, splash pads, museums, farms, cultural and theatrical events, etc.
Community Networking: is a community-based program that assists individuals with getting involved in their local area. Activities can include but are not limited to volunteering, clubs, classes, or opportunities for continuing or higher education. Community Networking is designed to help the individual to develop natural supports within the community.
Admission Criteria
Eligibility Requirements:
• Age requirements vary by program and are 18 years of age minimum
• Intellectual / Developmental Disability or Traumatic Brain Injury
• Applicants who exhibit aggressive or dangerous behaviors may not be accepted
• If applicants experience seizures, they must be under control
• Applicants must be able to physically and emotionally care for their needs (to include but not limited to ambulation, eating, dressing, bathing and toileting) Unless 1 on 1 waiver services are available
• Applicants must be willing to participate in assigned program
Admission Procedure
• Applicants must be referred by an LME/MCO or other funding source that is contracted with The Workshop of Davidson, Inc. (Primary Referral Source: Partners Health Management 1.888.235.4673)
• Required information includes:
• Application
• Current physical exam
• Current psychological report with adaptive testing and definitive IDD or TBI diagnosis
• Health/Social/Vocational history
• Current sources of identification to include but not limited to: Valid NCID card, Social Security Card, Birth Certificate, etc.
• All applicants will be screened by the admissions committee. Those found ineligible may be referred to other agencies for services.
• All applicants are encouraged to visit the facility for a tour before admission
